Acute AWS is a term that describes the initial symptoms of alcohol withdrawal. These may occur anywhere from a few hours to about 5 days following cessation. The symptoms of alcohol withdrawal will typically begin within 6–12 hours of stopping drinking alcohol. The syndrome is due to overactivity of the central and autonomic nervous systems, leading to tremors, insomnia, nausea and vomiting, hallucinations, anxiety, and agitation. If untreated or inadequately treated, withdrawal can progress to generalized tonic-clonic seizures, delirium tremens, and death. The three-question Alcohol Use Disorders Identification Test–Consumption and the Single Alcohol Screening Question instrument have the best accuracy for assessing unhealthy alcohol use in adults 18 years and older. Two commonly used tools to assess withdrawal symptoms are the Clinical Institute Withdrawal Assessment for Alcohol Scale, Revised, and the Short Alcohol Withdrawal Scale. Patients with mild to moderate withdrawal symptoms without additional risk factors for developing cure for alcohol withdrawal symptoms severe or complicated withdrawal should be treated as outpatients when possible.

Finally, with the exception of patients with cardiac arrhythmias, electrolytes disturbances or previous history of AWS-related seizures, there is no evidence to support the routine administration of magnesium during AWS 49. The STT was proposed by Saitz et al. in 199426 where in chlordiazepoxide was given when CIWA-Ar ratings were eight or more. Patients who are non-verbal (e.g. stupor due to head injury) may not be suited for this regimen as they may not be able to inform the nursing personnel if they were to experience any withdrawal symptoms. Despite increased focus on identification and treatment of AUD in primary care settings,26 however, less attention has been given to prevention of hospitalizations (and rehospitalizations) involving AWS as a treatment outcome or quality measure. Delirium tremens represents the most severe manifestation (4th degree) of AWS, as the result of no treatment or undertreatment of AWS 6, and occurrs approximately in 5% of patients with AWS 6.
